Chicago Nasal & Sinus Center will directly bill the insurance plans with which we are currently contracted. Below is a list of insurance plans currently accepted by our office:

If you do not see your plan on the above list, please inquire when calling for an appointment.

A photocopy will be made of a current insurance card and will be kept on file, but if authorization from a primary care physician is required, it must also be brought to the office or be on file at the time of a patient visit. Most managed care plans have specific rules that must be followed by both the patient and the physician in order for coverage to be applied.

We ask that patients notify our office of any changes in their insurance coverage as soon as possible.
