Appointments

Appointments can be made by calling (312) 372-9355 or using the NeedMyDoctor® link. After-hours, appointment requests may be left on our voice-mail system and calls will be returned the next business day; please do not use the answering service to request an appointment as this is for existing patient emergencies only.

Your Initial Visit Preparation: On your first visit, you will need to complete a Medical History, the Rhinosinusitis Disability Index and a Patient Registration form. Download and complete these forms ahead of time to facilitate the check-in process.

Please bring copies of any records of prior testing or treatment (CT scans/reports, operative reports, allergy testing results, etc.) to help make the most out of your consultation time with Dr. Pritikin. Please bring a current list of any medications that you take on a regular basis.

When you check-in, we will also ask to scan images of your:

  • Driver's License (or other major ID)
  • Medical insurance card and referral/authorization (if HMO)

Cancellation Policy: We respect the value of your time and ask the same of you. Therefore, we request the courtesy of a 24 hour advanced notice for rescheduling or canceling your appointment. If this request is not honored, we reserve the right to ask for pre-payment of the consultation fee by credit card prior to rescheduling another appointment for you.

Chicago Nasal and Sinus Center is located at 111 W. Washington in the heart of the Loop and just across the street from City Hall. Cross-streets are Washington & Clark to the east and Washington & LaSalle to the west.

Parking

For your convenience, discounted parking is available at 217 W. Washington, just 1-1/2 blocks from our office. Parking is $10 for 2 hours with validation.
